Description
The overriding ambitions of the RAPHAEL consortium are to:
• Improve the quality of life, quality of death and autonomy of patients with heart failure and their family caregivers by means of an integrated palliative care approach in heart failre care, supported by home monitoring of signs, symptoms and concerns.
• Improve the cost-effectiveness of heart failure care by reducing hospitalisation and readmission rates whilst promoting quality of Life.
• Offer universal and timely access to palliative care for all patients with heart failure in Europe, incl. those in more remote and rural areas, by means of a home-based, data-driven remote monitoring palliative care program.
